Guidelines for Care Before and After Surgery

Like most everything else, standard procedures and guidelines for care before and after surgery have evolved in an effort to promote and speed recovery.  Dr. Charla Fischer, an orthopedic surgeon at NYU Langone Spine Center in New York City, suggests you think of surgery as you would a marathon or race.  She says, "The surgery itself is just the first part of the race – there is also the recovery after.”  So what you do before a planned surgical procedure and what you do after can all be part of a better outcome and even includes diet and exercise.   

One example is a change in the way a person is treated after surgery in many hospitals.  Now, rather than limiting patients to ice chips and intravenous fluids, they are served a meal.  In addition, people are encouraged to get up and move as soon as the night following surgery, which appears to hasten recovery.  However, doctors warn that people should be screened before surgery for conditions such as malnutrition, anemia, diabetes and pain, and if it is found they have any, it should be addressed.  

Also, find out what exercises would be helpful and doable before and after surgery, and what is not recommended.  Ask about your hospital’s rehabilitation program or the availability of a physical therapist.   After surgery, stay hydrated and be sure to eat a protein-rich diet and lots of fresh vegetables.  Another thing that will aid your postoperative recovery and overall health is to stop smoking.   

As a bigger percentage of surgeries are in people over 65 years of age, your doctor will let you know the type of screening that is required before surgery.  It will most likely include things such as cardiac health, functional status and history of falls, frailty and other factors.

How to Maintain and Improve Your Memory

One of the first things they recommend is to never stop making “physical activity a priority as it optimizes blood flow to the entire body, including the brain.  An adequate supply of blood and oxygen to the brain helps to keep our memories sharp.”  Researchers explain that the type of exercise is also important and recommend a minimum of 30 minutes a day of cardio exercise such as running, power walking, biking, or swimming.  However, before beginning any new exercise regime, be sure to check with your healthcare provider.  

In addition, continuing to learn new things can improve our memory as studies have found a correlation between challenging our brain and maintaining brain health.  This could include activities such as playing an instrument, starting a new hobby, learning a language, or just broadening your reading habits.

Stretching Exercises are Easy to Learn and Relatively Easy to Do

Health professionals say it is important to incorporate stretching exercises into your routine especially the older we get as our flexibility naturally decreases. As joints become stiff, and tendons and ligaments tighten up, we become more susceptible to injuries as well as chronic pain and poor posture. In fact, studies show men’s flexibility drops sharply at around 70 years of age. 

Stretching exercises are easy to learn, relatively easy to do, and should not take more than five to ten minutes. Urvashi Chogle, a physical therapist at Harvard-affiliated Spaulding Rehabilitation Network, says the shoulders, hips, knees, and spine lose the most flexibility.  He explains, "This makes many everyday movements more difficult, such as reaching overhead, squatting, twisting, and bending over." 

Of course, before you begin any new stretching exercises, you need to consult with your healthcare provider. He or she may have suggestions on how to safely get started or direct you to your local community senior center or other organizations for classes. Hearing Loss: A Common Problem for Older Adults

The National Institute of Health reports approximately one in three people between the ages of 65 and 74 has hearing loss and nearly half of those older than 75 have difficulty hearing. Hearing loss causes someone a range of problems including difficulty understanding and following a doctor’s advice, responding to safety warnings, and hearing doorbells, phone calls, and alarms. Another all-too common problem is it can be hard to follow conversations with family and friends, and many start avoiding situations where they may be frustrated or embarrassed. 

Medical experts advise asking your primary care physician for a referral to an otolaryngologist, an audiologist or a hearing aid specialist. An otolaryngologist is a doctor who specializes in diagnosing and treating diseases of the ear, nose, and throat. They will try to determine the cause of your hearing loss and treatment options. They may refer you to an audiologist who is trained in measuring the degree of loss as well as recommend treatment, which may include fitting you for a hearing aid. There are three different types of hearing aids and it may take some trial and error to find one that works for you (most can be purchased for a trial period).  

In some cases, doctors may recommend Cochlear implants, small electronic devices surgically implanted in the inner ear that help provide a sense of sound to people who are profoundly deaf or hard-of-hearing.  Other treatment options include hearing assistive devices for your phones to amplify sound; closed circuit systems for churches or theaters; and lip or speech reading classes.

Do You Feel Memory Problems are Interfering with Your Life

Do you feel as if memory problems are interfering with your life or have family members started to notice? Medical experts say there may be a distinction between a person noticing the memory loss and others becoming aware of it as this could signal a bigger problem and be an early sign of dementia.  

The Harvard Medical School explains the best place to start understanding the problem is to talk to your doctor. While they may not have the proper training to do extensive screening for MCI (mild cognitive impairment), they can rule out other things causing your memory loss such as medications you take or a recent head injury. Steps you can take on your own to improve cognitive function include getting enough sleep each night, eating a healthy diet, exercising regularly, and reducing stress.

After your own physician has completed an examination, and nothing appears to explain your memory loss, it is time to see a specialist. They, or your local Alzheimer’s Association chapter or hospital should be able to recommend a behavioral neurologist or a geriatric psychiatrist. He or she will probably start with a series of written, on the computer, or oral tests to “assess memory, reasoning, attention, language, visual functions, motor functions, and social functions (such as empathy and knowing how to behave appropriately in social situations).”

They will check for “abnormalities in muscle strength, coordination, reflexes, senses, eye movement, and the pupils' reaction to light, which can signal abnormalities in specific areas of the brain.” In addition, the doctor will check for Parkinson’s, a stroke or tumors, and will probably schedule a hearing and vision test. If indicated, you may have advanced testing such as PET scans or your cerebrospinal fluid (CSF) biomarkers checked for signs of Alzheimer’s disease.

The Elderly are More at Risk for Malnutrition

The National Council on Aging (NCOA) cautions that elderly adults with chronic diseases such as diabetes, high blood pressure, and heart disease among others, are more at risk for malnutrition.  Bad eating habits, loss of appetite, lack of access to healthy foods due to an inability to get out and shop, or a shortage of money, only add to the problem. 

The NCOA also wants seniors to know that poor nutrition not only makes it harder to manage chronic diseases but may actually make you more vulnerable to coming down with them.  Another problem is it can weaken your immune system leaving you susceptible to the flu, colds and other viruses.  Moreover, poor nutrition can hamper one’s ability to recover from surgical procedures, heal wounds, and increases the risk for infection or falls.

Another Important Vaccine for Older Adults

Currently Shingrix is the only vaccine approved for adults 50 years and older to protect against shingles. This vaccine is important as the Centers for Disease Control and Prevention reports nearly one in three people in the U.S. develop shingles at some point, and every year an estimated 1 million people are diagnosed with the disease.  

Symptoms of shingles include pain, rash, itching, and at first fluid-filled, then eventually crusting blisters. People can additionally suffer from a hypersensitivity to touch or light as well as headaches and fevers. The disease can last anywhere from a few weeks to several months and pain is generally most intense in the first few weeks. Some individuals may require pain medication to sleep or get comfort.   

When Shingrix first came on the market in 2017 so many people wanted it there weren’t enough dosages to meet demand. Now manufacturer GlaxoSmithKline reports there is plenty of supply available for anyone who wishes to get the shot. For many people who put off medical care during the COVID-19 pandemic, now may be the time to protect yourself against shingles.

The vaccine requires two doses given any time between two to six months; and studies show it to be highly effective against the shingles in both older healthy adults and frailer ones. “In a large multicenter study of approximately 27,000 people vaccinated with Shingrix or a placebo, effectiveness ranged from about 90% in frail participants to nearly 96% in non-frail participants.”

The National Institute of Health reports approximately one in three people between the ages of 65 and 74 has hearing loss and nearly half of those older than 75 have difficulty hearing. Hearing loss causes someone a range of problems including difficulty understanding and following a doctor’s advice, responding to safety warnings, and hearing doorbells, phone calls, and alarms. Another all-too common problem is it can be hard to follow conversations with family and friends, and many start avoiding situations where they may be frustrated or embarrassed. 

Medical experts advise asking your primary care physician for a referral to an otolaryngologist, an audiologist or a hearing aid specialist. An otolaryngologist is a doctor who specializes in diagnosing and treating diseases of the ear, nose, and throat. They will try to determine the cause of your hearing loss and treatment options. They may refer you to an audiologist who is trained in measuring the degree of loss as well as recommend treatment, which may include fitting you for a hearing aid. There are three different types of hearing aids and it may take some trial and error to find one that works for you (most can be purchased for a trial period).

Do You Have Trouble Sleeping?

Whether it is chronic or happens every once in a while, most of us have had a restless night and woke up feeling tired. For some it is hard to fall asleep, while for others, it is difficult to stay asleep for the whole night. Sleep experts offer several tips for a restful night’s sleep and a more productive, positive day.  

The American Academy of Sleep Medicine (AASM), Centers for Disease Control and Prevention (CDC), and the Sleep Research Society (SRS) explain sleep is as important to a healthy lifestyle as good nutrition and regular exercise. To become a better sleeper, it helps to know something about the different components of sleep.   

The current thinking for a normal night’s sleep is between seven to nine hours a night, or during a 24-hour period. They say to determine your optimal sleep amount, experiment for a few weeks by going to bed each night when you get tired and waking naturally in the morning. Another thing is to deal with health issues, which may interfere with your sleep, like restless leg syndrome, sleep apnea, insomnia or other sleep disorders.  

The Mayo Clinic says it helps to go to bed at the same time each night and to wake up at the same time. On weekends try not to change your schedule by more than an hour. They also caution to not eat too heavy of a meal too close to bedtime and avoid nicotine, caffeine and other stimulates several hours before bedtime. The same goes for alcohol as it can make you fall asleep easily but causes you to awake in the middle of the night.  

Get plenty of exercise during the day but avoid doing it right before bed. Another tip is to create a calming atmosphere in the bedroom; and remember our smart devices emit glowing blue lights that experts say can disturb our circadian rhythm.

Increase Flexibly with Stretching Exercises

If you are a senior who wants to start exercising again, you may not know how to begin.  For one thing, you probably realize you are not as fit as you used to be, and are not up to the same degree of physical activity that you once did.  And of course, before you begin any new form of exercise, you will need to check with your doctor to make sure you are fit enough, especially if you have osteoporosis, an injury, or balance issues.  

Once you get the go ahead from your healthcare provider, stretching is a good place to begin.  In fact, studies show that as we grow older our flexibility decreases by up to 50 percent in some joints.  Another good thing about stretching is many exercises are appropriate for any level of fitness including ones that can be done sitting in a chair.  Stretching can help you maintain your flexibility and mobility as well as loosen up some of your stiffness in your back, neck and shoulder muscles.

Another plus about stretching exercises is that most of them do not require special equipment and can be done anywhere. The Silver Sneakers website recommends that as you stretch, “to breathe deeply, and go slowly. Listen to your body, and never force a movement that causes pain.”  They remind seniors it is okay if they cannot bend very far as over time they will loosen up some.  Safety is always important and you do not want to hurt yourself.  Other exercises especially good for seniors include chair yoga, resistance band workouts, walking, and dumbbell strength training.
