Think you are too old to pump iron?  Research shows that for the majority of seniors who still do exercise, their regimen only includes aerobic exercises such as walking, biking, jogging or swimming.  While these types of exercises are important to our overall health, an article published by the U.S. News & World Report explains why seniors can benefit from strength training.  They state that fitness professionals as well as healthcare providers report everyone from 18 to 80 needs to do both cardiovascular and strengthening exercises. 

Research suggests that the benefits go beyond just building better tone and an improved shape as studies link “not just aerobic exercise but strength training with a reduced risk of death from all causes.”  This is why the American Heart Association and American College of Sports Medicine recommends that even older adults follow their guidelines and do some level of strength training at least two days a week.  As Dr. Jennifer Kraschnewski, an associate professor of medicine, pediatrics and public health sciences at the Penn State College of Medicine points out “ very good evidence [is available] to show that 80-year-olds are just as able to improve their muscle mass as 50-year-olds. And this can be done reliably over a very short period of time – something as short as two to three months."

Have You had a Virtual Telemedicine Healthcare Visit Yet?

Lots of changes are going on regarding the way we are leading our day-to-day lives including visits to our healthcare providers.  Chances are good that if you have a scheduled visit to your physician, you may be told that it will be a virtual telehealth visit as many physicians have substantially cut back on in-person office visits.  

While the concept may seem new, you may be surprised to learn that the Lancet medical journal first discussed the possibility of using the telephone as a way to lessen unnecessary office or home visits in 1879, three years after Alexander Graham invented the telephone.   Now with the outbreak of Covid-19 and telehealth visits quickly gaining popularity, experts offer some guidance on what to expect and tips to make sure you are prepared for your “visit.”  

First, in order to participate in a telemedicine appointment, you need a telephone line, a high-speed internet connection, and video access through a smart phone, tablet or computer.  Also your provider needs to tell you what app to download before your visit.  

Moreover just like a regular in-office visit, doctors recommend making a list of topics you wish to go over with your healthcare provider.  Dr. Gary LeRoy, an associate professor of family medicine at Wright State University and president of the American Academy of Family Physicians, advises patients to ask themselves, “‘why do you need to be seen today; and what's the biggest priority?”  He suggests keeping a symptom diary of when the problem first developed and what seems to aggravate or alleviate the symptoms, etc.   Also make your call in a quiet place where you will not be interrupted, and have on hand any devices you use including thermometer, blood pressure cuff, glucometer, etc.  

If you need a mammogram, colonoscopy, or other procedures that require in-person care, experts recommend postponing them until we are in a better place in terms of the current health crisis.  However they stress that emergencies are still emergencies including “sudden chest pains, weakness in one side of the face or body, or a sudden thunder-clap headache. These are all red-alert symptoms that could signal a heart attack or stroke,” and require immediate medical attention.

How to Cope with Anxiety

The fact that people are experiencing increased levels of anxiety is one of the many consequences of living in such uncertain times.  In fact one poll from the Washington Post and ABC News in late March, says that “77 percent of American women and 61 percent of men were reporting personal stress.”  The poll also found that 69% of Americans are worried about family members, and even more worry that the economic impact will be greater or worse than the 2008 Recession.   

So how do we deal with the new normal?  Experts stress that we need to remember to take care of ourselves, both our bodies and our minds.  As we work from home, take care of our families full-time, and deal with the worries of the world; don’t forget to take the time to check in and listen to what your need.   

Ben Crocker, MD, a medical director for a large primary care practice and a healthy lifestyle advocate, reminds us that it is okay to mourn for what we have lost.  He says it is normal to feel sad about not being able to see family members or friends or unhappy about not being able to attend worship services or a favorite activity or class.  When we acknowledge loss, it helps us to feel more grounded with our reality.  Dr. Crocker suggests we intentionally add some ‘shutdown’ time in our schedule for “meditation and quietude.”

Other tools to help cope with anxiety include eating a nutrient-rich diet and making time to take a walk, ride your bike or exercise at home.  Also remember it is important to get enough sleep each night as well as to connect socially with loved ones.  Finally don’t get caught up in just focusing on the negative but rather take some time out each day to be grateful for what we do have.

One mistake many people make when they or a family member is diagnosed with Alzheimer’s is to think their life is over.  In reality, a person can live a productive life for many years.  Adopting a healthy lifestyle is important and should include a nutrient-rich diet and routine exercise.  Experts also note it is important to stay socially connected and engage in activities that challenge the brain.  As medication is more helpful in the earlier stages, getting a proper diagnosis is vital.     

People also fear that if a parent had Alzheimer’s, they will get it.  The facts are that “familiar Alzheimer’s disease accounts for less than 5% of all cases of Alzheimer’s disease.”  Furthermore, remember Alzheimer’s is not only a disease of the elderly.  While there are many fewer cases, it does occur in people as young as their 40s and 50s. 

Another common myth is the belief that dental amalgam fillings containing a small amount of mercury caused the disease.  Studies have shown no such correlation between the two.  Others believe aluminum pans are responsible, and while a few studies suggest a small possibility, most experts think the link is questionable.

The Effects of Watching Too Much Television

For years research has suggested children should have limits placed upon their daily screen time, and now a large study suggests the same may be true for older adults.   A published report from the University College London in the United Kingdom indicates that older adults who spent at least 3.5 hours a day watching television experienced a greater decline in verbal memory.  The study’s data was taken from the English Longitudinal Study of Ageing (ELSA), which included 3,662 participants age 50 years and older. 

Participants answered researchers’ questions in 2008-2009 and then again in 2014-2015 regarding the time they spent watching TV.  Afterward they were given a series of tests that measured verbal memory and fluency.  Results showed adults who watch 3.5 or more hours of TV daily had an “average decline of 8–10 percent in word- and language-related memory over the 6 years the study covered. This is compared with a lower 4–5 percent average decline in those who watched fewer hours of TV per day over the same period.”   For the verbal memory tests, participants were asked to memorize and then recall lists of words within a given time.  The verbal fluency tests required them to list as many examples, such as animals, they could in a given amount of time.  The semantic fluency of the study’s participants did not seem to be affected. 

It was noted that most of the studies involving the effects of TV watching have been done on children and little to no studies on older adults.  One of the study’s co-authors, Dr. Daisy Fancourt, who works in the Department of Behavioral Science and Health says, "Much less attention has been paid to the effects of television viewing at the other end of the lifespan, despite it being hypothesized for over 25 years that watching excessive television could contribute to the development of dementia."

It is Important to Stay Connected with Your Loved Ones

If you have elderly loved ones who are living alone or in senior living communities that have halted visits, the AARP offers several things you can do to help them stay socially connected and avoid feeling isolated.  Remember one of the most important things we can do is to let them know they are loved and missed.  

The easiest way you can do this is through frequent phone calls, but also don’t forget other ways of communicating such as sending a letter or greeting card through the snail mail reminding them you are thinking of them.  Include the kids by having them draw a picture for grandma or grandpa or if they are old enough, have them also write a letter.  

It definitely makes it more personal to stay in touch if your parent has a computer or a smart device that lets you facetime or skype on a regular basis and the AAPR has some ideas of how to use video-calling to keep it fun.  One suggestion if your kids are young is to arrange to have grandma or grandpa read books to them over a video-call (if they do not have suitable books, go online and send them some). Or if your child is learning to read, have them read to their grandparents.  

Another thing you can do is schedule a meal to be delivered to them from a local carry out place or from a service such as Grubhub, and settle down and enjoy sharing a meal with them while video chatting.  In addition, conversation starters can include watching the same movie or documentary and then spend time talking about it, or setting up a game or chessboard and playing the game together over the phone.

We are Vigilant in Our Approach to the Coronavirus Outbreak

At the MorningStar of Santa Fe senior living community, we are staying vigilant in our approach to the outbreak of the new coronavirus (Covid-19) disease.   Our highest priority is to the health and safety of our residents, their families and our staff members.   

We are doing all we can to achieve this including instituting a policy that temporarily suspends non-essential visitors and vendors from entering our home.  MorningStar is also keeping up-to-date and following closely, and in many cases exceeding, guidelines set by the CDC to ensure the health and wellbeing of everyone in our home.   

Some of these actions include having all assisted living residents dine in their suites with the exception of those with an Individualized Service Plan that requires assistance with eating.  They will be served in the dining room, widely spaced from one another. Our memory care residents will dine as usual but they too will be spread out as far apart as possible.  Other steps include suspending all non-essential visits (medical personal and end-of-life situations are the exception), eliminating all resident outings, and cancelling all events and outside entertainment.  

All of our team members have received training on infection control with an emphasis on hand sanitation including information from the CDC on COVID-19 and the uniqueness of this viral strain.  We have ordered extra disinfectants and other cleaning supplies as we as modified and increased the frequency of our cleaning practices throughout our home.

Power of Attorney for Health Care

A power of attorney is simply a directive that names someone you trust to act as your agent in the event that you are unable to speak for yourself.  A medical power of attorney, also called an “advance directive,” names someone to specifically act on your behalf in regards to any medical matters.  

If you or a family member is suffering from Alzheimer’s or another dementia-related disease, it is especially important to have a medical power of attorney.  According to the Alzheimer’s Association, the kinds of medical decisions covered by a power of attorney for health care includes: doctors and health care providers, types of treatment, and care facilities.  The designated medical power of attorney may also need to make some end-of-life decisions such as issuing a “do not resuscitate” (DNR) order or not consenting to a feeding tube when a person reaches the later stages of Alzheimer’s or dementia.  

The website, WebMD suggests that a person entrusted to be your medical power of attorney should be someone “who is not intimidated by medical professionals and is willing to ask challenging questions,”   They also add that the person needs to be able to put aside their own feelings about a medical option and be ready to carry out your wishes.  

Drawing up a legal power of attorney for health care does not require an attorney, and if it is appropriate, the same person can be named as your financial and medical power of attorney.  As forms are state-specific, be sure to use the right form for your state. Once you have filled it out, it will probably need to be witnessed, and in some cases may also need to be notarized.  When you have completed the process, let your family and anyone else involved in your care, including your doctor and hospital have a copy of the form.

Exercise is Beneficial for Everyone

Research has shown over and over again that physical exercise is beneficial to older adults regardless of their age and health limitations.  If you are an older adult who is already physically active, you may be helping to “prevent problems like a lower maximum heart rate, reduced muscle mass, bone loss, stiffened tendons and ligaments, and some joint or nerve problems.”  Experts recommend at least 30 minutes of physical activity daily or 150 minutes of moderate-intensity aerobic activity per week.

If you are an older adult with some health limitations, the U.S. Department of Health and Human Services and the American Heart Association offer some useful tips for safely benefiting from physical activity.  Just remember, before you start any new exercise regimen, consult with your health care provider. Also while you may not be able to attain the recommended 150 minutes a week, experts say you should try to engage in as much physical activity as you can.  

Adjust the intensity of your physical activity to your fitness level, and find activities you can do all year round and enjoy.  Walking is considered to be a good exercise for older adults as it can easily be adjusted to one’s fitness level by adjusting the length of the walk and pace.  Also remember to wear comfortable clothing as well as properly fitting footwear. Walking with someone makes good sense for safety reasons and provides companionship.  

For seniors at risk of falling, it is important to do exercises to improve your balance.  Most of the recommended exercises are easy-to-do at home and can be done daily. You will want to start any new activity slowly and increase your progress gradually.  Another thing to remember is to drink plenty of water throughout the day and during your exercise activity. Keep in mind that as we age our thirst sensation diminishes, and by the time you feel thirsty, you may already be partially dehydrated.
